Care Coordination

Patients in our Care Coordination program work closely with their Preferred Provider and the clinic's Care Coordinator to monitor and treat their chronic conditions.

Participants in the program are encouraged to make timely follow-up appointments for office visits, blood work and other tests to ensure thorough and appropriate management of their particular condition.
