Patient Billing Policies:

  • All copays are due at the time of service. If you are unable to meet your payment obligations we can reschedule you to a more suitable time.

  • We accept multiple insurance plans; patients must verify coverage directly with their plan and the patient accepts ultimate responsibility for payment.

  • The patient is responsible for providing necessary information for insurance claims and for working with our office to make payment arrangements when necessary.

  • All charges are ultimately the patient's responsibility, regardless of insurance coverage.

  • Patients who have no insurance are eligible for a cash payment discount. You will be asked to pay at the time of your appointment. If you are unable to meet your payment obligations we can reschedule you to a more suitable time. We appreciate your understanding.

  • If you have any questions regarding your bill, please call our Billing Department at 763-398-5233.

Questions about your bill from Quest Diagnostics Lab?

  • AALFA Family Clinic uses Quest Diagnostics Laboratory for some lab testing. You may receive a bill from Quest Lab as well as from AALFA Family Clinic for your lab work.

  • The patient is responsible for any balance due to Quest after the insurance has been processed.

How do I know if you take my insurance?

  • Contact your insurance company's member services (usually listed on the back of your insurance card) directly to verify your coverage and benefits.

  • We do accept many insurance plans, but cannot guarantee that your insurance will cover our services. The patient is responsible for verifying their individual policy benefits and coverage.

I don't have insurance. Can I still be seen?

  • If you have no insurance, you will be asked to pay at the time of your appointment.

  • If you are unable to meet your payment obligations we can reschedule you to a more suitable time. We appreciate your understanding.

How much will my visit cost?

  • Price Transparency

  • Any pricing quoted prior to a visit is an estimate and subject to change depending on the actual services performed within the visit.

  • The patient's amount due for any office visit can vary significantly. Final pricing depends on numerous factors including but not limited to: the type and extent of the visit, insurance coverage and benefits, additional services performed, and lab work. When you call to request an estimate, please be aware that the amount due for your visit may change based on actual services provided.

Can I apply for financial assistance?

  • If you require a payment program, you must make arrangements through our billing department. We require half of your patient responsibility up-front. Your subsequent payments will be determined based on the remaining balance.
